Duties of the Social Worker:
Undertaking comprehensive assessments of individuals and their families in line with the National and Local criteria for services
Developing, coordinating and delivering comprehensive and appropriate care packages to service users.
Engaging with the individuals and their families, working in a person-centred approach to empower them and safeguard their welfare.
Participating in multidisciplinary meetings, and collaborating closely with other agencies to ensure a harmonised approach to care.
Working effectively with individuals who have complex needs, mental and physical and behavioural, promoting their health and well-being whilst ensuring their safety at all times.
Responding promptly to crisis situations and conducting immediate risk assessments when necessary.
Maintaining up-to-date and accurate records, in line with legal requirements and departmental policies.
As a Social Worker you MUST be:
Social Work England Registered
2 years post qualified experience in a statutory setting
Eligible to work in the UK
A full UK drivers license holder
