Responsibilities of this role:
Conduct comprehensive assessments of adult patients' physical, emotional, and social needs to develop a deep understanding of their individual circumstances.
Coordinate and facilitate safe and timely discharge plans.
Provide advocacy and support to patients and their families.
Conduct risk assessments and implement safeguarding measures to ensure the well-being and safety of vulnerable adults during the discharge process.
Respond to urgent requests to visit anywhere in the borough.
Requirements of this role:
Must be a Qualified Social Worker, registered with Social Work England.
Must has at least 2 Years Post Qualifying experience.
