Are you a dedicated and compassionate social worker with a passion for enhancing community wellbeing? Randstad is seeking a skilled Community Wellbeing Social Worker to join our team in North Tyneside. This is an excellent opportunity to make a significant impact in the community by supporting individuals and families to achieve better health and social outcomes.
Key Responsibilities:
Conduct comprehensive assessments of clients' needs, strengths, and risks.
Develop, implement, and review personalized care plans in collaboration with clients, their families, and other professionals.
Provide direct support and interventions to improve clients' physical, emotional, and social wellbeing.
Advocate for clients and ensure they have access to necessary resources and services.
Work collaboratively with local agencies, healthcare providers, and community organizations.
Maintain accurate and up-to-date case records and documentation.
Participate in team meetings, supervision, and continuous professional development.
Qualifications:
Degree in Social Work (or equivalent) and registration with Social Work England.
Proven experience in a social work role, preferably within community wellbeing or similar settings.
Strong understanding of relevant legislation, policies, and procedures.
Excellent communication, interpersonal, and organizational skills.
Ability to work independently and as part of a multidisciplinary team.
